This questionnaire takes approximately 25 minutes minimum and an hour maximum to complete.

If filling out questionnaires tends to cause you feelings of anxiety or overwhelm, here are some ways that you can support yourself for success.

• Make sure to take the questionnaire in a calming location to minimize overstimulation or distractions.
• Don’t force yourself to take it right away.
• Perhaps wear headphones with you to block out noise if you need to.
• Have someone available as support to talk about the questions to.
• Feel free to take breaks & complete it in chunks if you find yourself tired/overwhelmed.
